If you are an accredited investor, as defined below, interested in future investment opportunities in VAIREX, please fill in and send the following information. You will be notified of any future investment opportunities open to new investors.
To qualify as an accredited investor, you must meet at least one of the following criteria:
1. Have an individual net worth or joint net worth with your spouse in excess of $1,000,000.
2. Have had an individual income in excess of $200,000 in each of the previous two years and a reasonable expectation of an individual income in excess of $200,000 in the year of investment.
3. Together with your spouse have had a joint income in excess of $300,000 in each of the previous two years and a reasonable expectation of a joint income in excess of $300,000 in the year of investment.
4. Be a director and/or an executive officer of the Company as such terms are defined in Regulation D promulgated under the Securities Act of 1933, as amended.
